What are the most important health and health care issues in Alachua County? What do you think about the quality of health in Alachua County?
The Florida Department of Health in Alachua County, University of Florida Health Shands Hospital, and WellFlorida Council, the local health planning council, are inviting the Alachua County community to answer a community health needs assessment survey between Monday, January 14, and Saturday, March 2, 2019.
Alachua County Library District is helping these organizations reach our local community by promoting this survey and encouraging more Alachua County residents to participate in the survey.
Community leaders will use answers from the survey to build a healthier community. Answers from the survey will not be used to identify participants in the survey. The survey should take about 15-20 minutes to finish online, with 41 total questions.
To encourage participation, WellFlorida Council is using a raffle to give away ten (10) Wal-Mart gift cards worth $30 each. To enter the raffle:
• You must be at least 18 years old to participate.
• Answer all questions on the survey.

Please answer the survey only once. Completing more than one survey will not increase your chances to win a gift card.
